How does ProLift know I made the referral?

The referred customer must provide your full name and mailing address at the time they schedule or receive an estimate.

When do I get paid?

After the job is completed and full payment has cleared. Checks are typically mailed within 30 days.

Is there a limit to how many people I can refer?

No. There is no cap on referrals or rewards.

Can I refer myself or someone in my household?

No. Self-referrals are not eligible.

What if my referral cancels or doesn’t pay?

Referral rewards are only issued for jobs that are fully completed and paid in full.

Do I need to submit anything to make a referral?

No. The referred customer simply needs to provide your full legal name and mailing address when they schedule or receive an estimate.

What if more than one person claims the same referral?

Referral rewards are issued to the person whose name and address are provided first by the referred customer. Only one referral reward is issued per job.

Can a business or property manager be referred?

Yes, as long as they are a new customer and meet all referral requirements.

What if my referral was already talking to ProLift Concrete LLC?

If the customer previously contacted ProLift Concrete LLC, requested an estimate, or received services, the referral will not qualify.

What happens if the job is partially completed or disputed?

Referral rewards are only issued after the job is fully completed and paid in full. Disputed, canceled, or partially completed jobs do not qualify.

Can referral rewards be applied as a discount to my own job?

No. Referral rewards are issued by check only and cannot be applied as service credits or discounts.

How will I know if my referral qualified?

You may contact ProLift Concrete LLC to inquire about the status of a referral once the job has been completed and paid.

Terms and Conditions

These Terms & Conditions (“Terms”) govern participation in the ProLift Concrete LLC (“ProLift,” “Company,” “we,” “our,” or “us”) Referral Program (“Program”). Participation in the Program constitutes full and binding acceptance of these Terms.

1. Eligibility
  • Participation is limited to individuals 18 years of age or older who are legally eligible to receive payment in the State of Illinois.
  • Referrals must be new customers, defined as individuals or entities that have not previously contacted ProLift Concrete LLC, requested an estimate, scheduled services, or received services at any time prior to the referral.
  • Employees of ProLift Concrete LLC and members of their immediate household are not eligible unless expressly approved in writing.
2. Referral Requirements

A referral is considered valid only if all of the following conditions are met:

  1. A referral is submitted to ProLift Concrete LLC.
  2. The referred customer schedules and receives an estimate.
  3. The referred customer books a service appointment.
  4. The service work is fully completed.
  5. ProLift Concrete LLC receives full and cleared payment for the completed service.
  6. At the time of scheduling or estimate, the referred customer provides the referrer’s full legal name and mailing address.

Failure to satisfy any of the above requirements renders the referral invalid and ineligible for payment.

3. Referral Reward
  • A qualified referrer will receive one (1) $50.00 check per valid referral.
  • There is no limit to the number of referrals a participant may submit.
  • Referral rewards are issued per completed and fully paid job, not per estimate, inquiry, or booking.
  • Referral rewards are non-transferable, have no cash alternative, and may not be substituted.
4. Payment Timing & Method
  • Referral reward checks will be mailed to the referrer using the provided mailing address.
  • Payments will be issued only after full payment for the referred job has been received and cleared.
  • Processing time may take up to 30 days following receipt of payment.
  • ProLift Concrete LLC is not responsible for delays caused by incorrect, incomplete, or outdated referrer information.
5. Tax Reporting, W-9 & Compliance
  • Referral rewards may constitute taxable income under federal and Illinois law.
  • If a referrer’s total referral payments reach or exceed $500.00 within a calendar year, additional information may be required for tax reporting purposes.
  • Referrers may be required to complete and submit IRS Form W-9 prior to the issuance of additional payments.
  • ProLift Concrete LLC may issue Form 1099-NEC or other required tax documentation in compliance with IRS and Illinois Department of Revenue regulations.
  • Failure to provide requested tax documentation within a reasonable timeframe may result in delayed, suspended, or permanently forfeited referral payments, at ProLift Concrete LLC’s discretion.
6. Verification, Discretion & Fraud Prevention
  • ProLift Concrete LLC reserves the right to verify all referrals.
  • Fraudulent, misleading, duplicate, or self-referrals are prohibited.
  • ProLift Concrete LLC retains sole and absolute discretion to determine the validity of any referral and whether a referral reward is payable.
  • Decisions regarding referral eligibility and payment are final and binding.
7. No Employment or Agency Relationship

Participation in the Program does not create an employment, independent contractor, agency, partnership, or joint venture relationship between the referrer and ProLift Concrete LLC.

8. Program Modification or Termination
  • ProLift Concrete LLC reserves the right to modify, suspend, or terminate the Program at any time, with or without notice, as permitted by Illinois law.
  • Referrals submitted prior to termination will be honored only if all Program requirements are fully satisfied.
9. Limitation of Liability

To the fullest extent permitted by law, ProLift Concrete LLC shall not be liable for indirect, incidental, consequential, or special damages arising from participation in the Program.

10. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Illinois, without regard to conflict-of-law principles.

11. Severability

If any provision of these Terms is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

12. Final Authority

ProLift Concrete LLC’s interpretation and application of these Terms are final. Participation in the Program constitutes acknowledgment and acceptance of these Terms.
